Community Health Needs Assessment
In the practice of public health, the community is the patient and the health of the community is watched and measured on a routine basis. This is done by looking at key population health indicators such as local disease rates, causes of death, and behavioral health determinants, like smoking or drinking rates. In Wisconsin, each local health department is required to regularly lead a community health assessment. The assessment findings are widely spread among community members to allow public health practitioners and their community partners to address priorities which may call for notice and change community health improvement plans.
Healthy Sheboygan County 2020 partnered with Aurora Health Care, Health and Human Services, Division of Public Health, Lakeshore Community Health Care, St. Nicholas Hospital, United Way of Sheboygan County and UW-Extension of Sheboygan County to complete the needs assessments below:
